HOW WILL I RECEIVE MY CTLE CERTIFICATION?
ASSET is an approved CTLE Sponsor. Completion of Approved Continuing Teacher and Leader Education (CTLE) Hour(s) Certificate, Sections ll and lll will be completed by ASSET and sent electronically. Please complete Section I and retain your copies for eight years. It is not necessary to send a copy of this form to the Office of Teaching Initiatives unless it is requested in the event of an audit or for use in obtaining an Initial Reissuance.

CAN I REGISTER FOR THE CONFERENCE WITH A PURCHASE ORDER?

Please note that cancellations will only be honored if received by Monday, March 8, 2021.
All cancellations received after this date will be non-refundable.
